Open Position: Service Coordinator – TBI/MSC

The Epilepsy Society of Southern New York is a human services organization providing services to individuals with epilepsy, TBI and other developmental disabilities. Our service area covers seven counties in southern New York, including Rockland, Orange and Westchester.

We are looking for a highly motivated, professional individual to provide case management services to our consumers who have had a traumatic brain injury or a developmental disability. This rewarding position as a Service Coordinator is suitable for a professional who is creative, compassionate and resourceful with excellent organizational skills and a strong commitment to helping individuals with epilepsy or other disabilities such as traumatic brain injury, mental retardation, cerebral palsy, etc. Responsibilities include intakes, development of service plans and referrals in a multi-county area. Monthly meetings with individual consumers are required to assess services needed and provided coordination of these services. This rewarding position is suitable for a professional who is creative, compassionate and resourceful with excellent people and supervisory skills and a strong commitment to helping individuals with disabilities.

A Bachelor's Degree in social work or related field is required. Experience working with disabled individuals necessary, TBI preferred. Also, experience in service coordination/case management necessary. This position requires a valid driver’s license, the ability to safely operate a car and access to a car to drive to consumer appointments.
